CONSENT FORM FOR COLLECTION AND USE OF PERSONAL INFORMATION

  2. All information collected is used within the scope of the above purpose and is not used for any purpose other than the purpose

  1. Required : first and last name, email address, mobile phone number

3. Retention and Use Period of Personal Information 

  1. Retention period: 13 months
